2. What form do I use when I want to change general information that FPDR has about me and when do I need to complete it?


FPDR needs confirmation in writing from a Member when there are any changes in a Member’s status.  This includes address or telephone number changes, name changes and any changes in your family status such as marriage, divorce, birth or death.
