Dear Friends,
The Oregon DHS Public Health Division, Office of Family Health requests that you complete a survey about health issues that affect Oregon women, mothers, children adolescents, and adolescents with special health needs. The Women's & Reproductive Health Section is housed in this unit.
Sponsor and Purpose: The survey is sponsored by the Oregon Office of Family Health in collaboration with the Oregon Center for Children and Youth with Special Health Needs (OCCYSHN). Survey responses will be used as part of the process for establishing public health priorities for Oregon families for the next five years and for distributing funds through the Title V Maternal and Child Health Block Grant.
Amount of Time: We expect the survey to take 10-30 minutes depending on how many sections you choose to complete.
Deadline: The deadline for completing the survey is August 17, 2009.
Forwarding the Survey: We would greatly appreciate it if you copy this information, along with the survey link, to your staff, colleagues, and/or partners. By forwarding the email, you will help ensure that we receive responses from a broad representation of people and communities throughout Oregon. We apologize in advance if you, or someone you know, receive this request more than once.
