I hereby submit an appeal for an alternative method or a modification of the requirements of the Building Codes of the City of Portland as outlined in the attached information. I shall, at my own expense indemnify, defend and hold harmless the City, its officers, agents, officials and employees against any and all claims, demands, actions and suits, including attorney's fees and costs brought against any of them arising out of or resulting from approval of any of these appealed items. I hereby further acknowledge that I am submitting this appeal as the property owner or the property owner's authorized representative.
