5011-01 FIRE BUREAU PERMIT RECORDS - TEMPORARY USE
Series is used to document permits as required by the Fire Bureau per City
and State Fire Codes. Series is created to document legal conditions of use
within City limits. Records include permittee information such as name, address,
etc. Records include location for use, type of use allowed, conditions,
agreements, insurance information, permit number, state permit number ) if
required), fee, etc. Examples of permits include retail fireworks permits,
firework display permits, vending permits for using liquid fuels, blasting
permits, public assembly permits, etc.

RETENTION: 2 years after expiration
5011-02 FIRE BUREAU PERMIT RECORDS - INSTALLMENT PERMITS
Permits are issued to document installation and removal of fire or storage
systems. Permit contains location, name installer/contractor, date, etc. Permit
may include plans, correspondence, calculations, tests, legal and financial
documents, etc. Examples of permits issued include sprinkler, alarm, fixed fire,
and storage tanks. Series includes Certificate of Fitness.
Record Copy? Yes
RETENTION: Permanent. Paper copy may be destroyed after it has been
microfilmed.
5011-03 TANK CARDS
Series is used to document the location of fuel storage tanks. Records
include location, contractor, permit number, work done, etc. Some records may
include site plans, schematics and other drawings. Series may include plans and
locations for other special hazards.

RETENTION: 5 years
5011-04 CERTIFICATE OF FITNESS TESTING RECORDS
Series is created to document individual tests for certification for
installing fire systems. Series is created to test individuals who seek
certification for Installation Permits. Record includes name of person, copies
of questions asked, test results, etc. Examples include tests given for fire
extinguishers, fixed systems, sprinklers, underground, clean agent systems, hood
and vents, and other similar tests.
Note: Some records contained in these files may be exempt from disclosure
under ORS 192.501 through 192.502. Consult your agency's legal counsel for
assistance before releasing information.
