5003-01 INJURY REPORTS
Series is created to review injuries to firefighters and to assist staff
during litigation. Records may include injury report, fund report, risk
management forms, workers compensation records, memos, correspondence and other
records concerning the injury. Note: Some records contained in these files may
be exempt from disclosure under ORS 192.501 through 192.502. Consult your
agency's legal counsel for assistance before releasing information.

RETENTION: 5 years. See also Personnel File (1004-01). Material of continuing
value should be transferred prior to destruction.
5003-02 OHSA 200 LOG REPORTS
Series is the hard copies of reports created from the Injury Log Database.
Records include name, case number, date, etc. Current examples of reports
include injury report, alpha list of employees, case number, OHSA log, OHSA 200S
report, etc.
Note: Some records contained in these files may be exempt from disclosure
under ORS 192.501 through 192.502. Consult your agency's legal counsel for
assistance before releasing information.

5003-03 OHSA INJURY LOG REPORTS
Series is used to summarize injury and occupational illnesses per OHSA
requirements. Records are both the database (created since 1987) and the hard
copies of the injury log. Information includes name, case number, date, short
description of injury or illness, loss time, restricted time, and other
information as required by OHSA.
Note: Some records contained in these files may be exempt from disclosure
under ORS 192.501 through 192.502. Consult your agency's legal counsel for

5003-04 VEHICLE ACCIDENT REPORTS
Series is used to investigate and review accidents concerning Fire Bureau
vehicles and apparatus. Records include risk management forms, correspondence,
review board minutes, investigative material, photos, etc.
